推薦状について教えてください

非常に基本的な質問で申し訳ありませんが、教えてください。

過去ログを見て私なりに理解したところでは、たいていの学校に対しては推薦状は、
・指定のフォームには、名前、肩書き＋レートの表（リーダーシップはEXCEPTIONALとかとういう評価表）を書く
・具体的な質問には、FORM上は"Please see the attached letter"とか"Please refer to the attached letter"とする
（この言い方でOKでしょうか？）
・LETTERを添付し、質問についてはその中でカバーする
ということになるかと思います

ここで質問です。

１．学校ごとに推薦状での設問が若干違うとおもいますが、漏れを防ぐために、ある程度事前にリサーチして、最小公倍数的なレターを作られているのでしょうか。それとも多少ヌケがあっても、大きな欠陥がなければ、そのまま他校に流用？

２．いくつかの学校は上記方法（別紙参照とする方法）は駄目だとも掲示板上で見かけました。この方式のOK／NGの判断は皆さんどうされてますか？たとえば、EMORYのインストラクションは以下のとおりでいいとも悪いとも書かれていません。この場合は、上記方式OKと判断していいのでしょうか？
